I'm familiar with the Kentucky mandolins line...
But, I just guessed at what it looked 'most' like...
Searched that on Google and lucked out...

Yes, Your Mandolin is A KM 675. I own one just like it. Mine looks as though the same person that filled out your label also feeled out mine.
